Overview
Home and Community Care provides home and community based health services to assist individuals of all ages with chronic and acute illness, disabilities, and the elderly to receive care in their homes within the Oneida Community. The program coordinates health and community support services and works in partnership with clients and families in care planning to maintain optimum health, well being and independence in their home and in the community.
Home and Community Care services are provided by Registered Nurses, certified Personal Support Workers and trained Home Support Workers. Program services include: client assessment service; case management; referrals and links with other community support services and external healthcare services; Loan Cupboard for short term needs (medical equipment). Personal Care support activities include; bathing, grooming, dressing, simple dressings, blood pressure/blood sugar and medication monitoring. Home support activities include light housekeeping, laundry, and meal preparation.
