The Community Health Program is delivered by Registered Nurses, Registered Practical Nurses, and Community Health Representatives. We provide a range of public health programs and services that revolve around health promotion, illness and injury prevention, and health maintenance. We focus on chronic disease management, prevention and screening of communicable diseases, and support of healthy lifestyles across the lifespan.
