Home and Community Care provides home and community based health services to assist individuals of all ages with chronic and acute illness, disabilities, and the elderly to receive care in their homes within the Oneida Community. The program coordinates health and community support services and works in partnership with clients and families in care planning to maintain optimum health, well being and independence in their home and in the community.

Providing safe and effective support to Oneida seniors age 55+ and allowing seniors to live independently in their own homes and community. Supports include: visitations for assessment, liason to service providers, minor home repairs, grass cutting, snow removal, access to traditional healer(s), medication pickup, and escorting seniors for appointments as needed.
